JAMM Instrument Loan Agreement

    Students borrowing instruments for non- JAMM purposes are asked to make a donation to JAM for the free use of instruments. Donations will be used to maintain and repair instruments. To make a donation, please click here.

    By submitting the agreement below student and their family understands and accept to the following terms:
    1. The student (and his or her parents or guardians) are responsible for returning the instrument and all of its parts and accessories to JAMM in the same condition that it was received.
    2. Instruments are checked out for home use and community ensemble rehearsal only; they are not to be taken to school during the school day.
    3. The instrument is not to be played outdoors.
    4. The instrument is not to be played by anyone other than the student who has borrowed it.
    5. JAMM reserves the right to ask for the instrument back or deny any musician or student the privilege of borrowing an instrument if the he/she has previously failed to uphold the terms of this agreement, if the he/she has not handled the instrument properly while in class, or if he/she has not shown the responsibility to protect the instrument while in his or her care.
    6. If the instrument is damaged, stolen, or if the student has lost the instrument or parts of it, he or she and his family are fully responsible for reimbursing the school for its loss.
    7. Contact JAMM immediantly if the instrument is damaged or cannot be found.
    8. In order to use a JAMM instrument, the student must be enrolled in a school or community orchestra and agrees to practice diligently according to the instruction of the music specialist.
